I may be over thinking this...but what do you use as a guide to create superbills? Is it by specialty or are some of the codes, like for office visits the same regardless??? LOST!!!

Most superbills are designed by specialty. I particularly like the ones that include all elements and even order diagnosis/procedures .. makes billing much easier

What is a good source to create one. I am working with a provider whose is outdates (old/deleted codes).
