I usually call first and see if they have a quick way to do it (some do) but then I also follow up with both a letter and W9 with the change of address. I then also do a follow up just to make sure about 7-10 days later.
Each carrier has their own methods. A few years ago this was a pretty painful thing but now it's pretty easy and straightforward. Lot less errors.

Just thinking if an 855I form would be needed for Medicare for updating a new practice location address. Because, recently we had a provider whose medicare checks were held up, as the provider started to bill out under a new practice address without proper notice. Later when following up with Medicare, we were advised to submit an 855I and it took about 45 days for processing that request.

Changing a practice location with Medicare takes a while.  Adding a practice location with Medicare usually happens much faster.

You might consider adding the new location as an additional practice location before the move, so that Medicare has both addresses in its system.  That way, they will accept billing from both locations.  After the move has been completed successfully, and things have settled down, you can submit a request to Medicare to delete the first practice location address.  Much less disruption to business doing it this way.
