Provider Change of Address

The 855I form will allow you to show we're practicing "here" until this date, and will be practicing "there" as of this date.  Just make sure that your dates are pretty firm.  Like Richard pointed out, it would be better to have an additional location just in case the move takes longer or there are delays.

I haven't done a Medicare one in a while but the last one I did was quick..they had told me 20-45 days but it was MUCH quicker :)  I remember just 5 years ago when changing an address was a nightmare :)

Yeah, in 2008 it took us 9 MONTHS to change our address!  NIGHTMARE!!!!!
