Podiatry Billing - CPT Code not payable by Medicare

Hi everyone! Have small medical billing business and just got 3rd account.  He is a Podiatrist using Athena Health just went live paper to electronic.

Question: Medicare kicked back claim stating:
   [CPT] Procedure code (CPT or HCPCS) is invalid or not acceptable for this payer.
Claim Billed out with:
99203 DX 728.71 - I don't understand why this is being rejected, Doctor is a "DPM" Hawaii
S9999 DX 728.71 ( Please confirm my findings on this. Medicare does not reimburse for state sales tax) I overrode this on original claim knowing that Medicare would reject this line as "not payable". Was it okay to do it this way?

 728.7 Other fibromatoses
    728.71 Plantar fascial fibromatosis
        Contracture of plantar fascia
        Plantar fasciitis (traumatic)
    728.79 Other

        Garrod's or knuckle pads
        Nodular fasciitis
        Pseudosarcomatous Fibromatosis (proliferative) (subcutaneous)

Any help would be great, live 8 days now and going crazy, there is little training available for Podiatry so I am a do-it-yourself learner. I work at due diligence before I come in here and ask you experts to show you the respect of effort . You are a almost a last resort and I know a reliable source of information.

I assume this is a Medicare Advantage Plan. Is the podiatrist credentialed with this plan? I haven't billed for Hawaii claims in years and yes, there is a tax on medical care (not just appliances) for those wondering. Sorry, wish I could help you.

Hi Jessica-

Congrats on your new client AND trying to teach yourself podiatry billing...not an easy task!

While I know nothing about billing for a provider in Hawaii, and about the sales tax requirement, I can offer one idea. If this is standard Medicare, I know that some MAC's will automatically reject an entire claim when one line or more on it is a HCPCS code that they don't cover, or should have been billed to DMERC, depending upon the situation.

Case in point, when my local MAC was WPS, I could bill certain non-payable HCPCS codes to them, along with payable CPT codes, and they would pay the CPT, and deny the HCPCS, which is what I knew would happen, and wanted to happen, so I could then bill the patient(per ABN, etc). But then my MAC changed to NGS, and I quickly found out that non-payable HCPCS codes on the same claim with payable CPT codes resulted in the entire claim being rejected. This could be what is happening to you, but because I have zero experience with the S9999 code, or your local MAC, I can't say for sure. Have you contacted Medicare for further clarification? That may be your best bet.

While I can't help you with this situation, feel free to message me if you need any other podiatry help. It is all I have billed for almost twenty years, and there isn't much I haven't seen/billed over the years. Except of course a DPM in Hawaii, and sales tax on medical care. ;)

Gosh, I am always just humbled by the knowledge base and willing to share from my medical billing community!

Kristen, that is the kindest offer and one I may have to take advantage of. Doctor has me in a "60 day trial" period and I need to perform! Please tell me how to message you! I do have other questions that are probably easy for you!
