Is it ok to still use modifier 59 or should we be using the sub modifiers XE, XS, XP, XU
When was the effective day to start using the sub modifiers?

It is ok to use modifier 59 if no other modifier that is more accurate is available. The following is a good explanation of the use of 59, XE, XS, XP & XU.
