I worked for a few psychiatrists (one was a forensic psychiatrist) and we did alot of these. His own attorney used to advise him to ONLY give what the attorney wanted. So the provider would speak with the attorney or DA, find out what they needed, and then send them a letter that just basically stated his fees per hour, travel, gas, etc.
If you will be tracking this, it's a good idea to setup a way to manage/track these just like you would patients.
I used to setup each legal practice or DA's office as "patients" and I used an internal coding system.
As for template, the best I can do is give you a short one on how this doctors letter would go to the attorney or DA office but AFTER he spoke with them and was advised by his own attorney.
