You have not provided enough information for anyone to be able to help.  Just saying "denied due to service render" does not give enough information for anyone to know the reason.  Is there a denial code?

OK, so without having the billed claim in front of me, and the eob it's very hard to help.  If that is truly all of the information given on the eob and the claim appears to have been billed correctly then I would recommend contacting Medicaid to ask if they can point you towards some documentation regarding 80305.  They may be able to provide a more detailed explanation on the denial.
