Medication Assisted Treatment (MAT)

Just wondering if anyone has a Superbill sample they might be willing and able to share for a physician's office providing Medication Assisted Treatment (MAT) for opioid dependency.  It would be really helpful to me and very much appreciated. :)  Thanks!

I'm sorry but I don't have anything specific to that.  What type of provider is it?  Medical Doctor?

Many of the codes will be the usual E&M codes, 99201-99215.  But unfortunately I don't have a sample superbill.
