I wish I could help you but your questions are still too vague.  What state are you in?

If a cpt code is inactive then you should use the next closest cpt code that describes the services rendered.

I *think* what the poster is trying to say is that those codes are not on the Medicaid Fee Schedule, because if Medicaid is primary, they are not services Medicaid pays for. If that is the case, there isn't anything that can be done. For instance, the 99490 is CCM, and Medicaid does not pay for that, and there is no other code that can be used. But I am not sure this is what the poster is asking, it is just my best guess.

I agree.  There is such a wide range of codes though it is confusing.  We have situations such as PT where NY Medicaid only allows one CPT code for PT so no matter what is done you have to bill it under that one code.  I wasn't sure with some of the codes because 36415 would be a covered service so if that code is not right there has to be another code.  We really need the poster to be more detailed/specific with their questions.
